Why can't I see a form element on my custom form?

All custom form elements are permissions based. 

In order for the element to be available for a form during form build out it has to be available to the user (owner) of the form.

1. You must have the appropriate permission(s) to access the custom form element.
   
   
   
   
2. The form MUST be set as PUBLIC (vs. private) Only public forms will show form elements.
